One fix, to a request the debug adapter left unanswered.

lyrdbg answers setExceptionBreakpoints. An editor configures exception breakpoints as part of its startup sequence — after the initialized event, before configurationDone — whether or not the adapter offers any filters to set. This one offers none and answered the request with an error, and a client that treats a failed configuration request as fatal never gets to configurationDone: the program stands there launched and never started, with no output and nothing to see.

There is still nothing to set and nothing to report, so the answer carries no body. What changed is that it is an answer. Nothing about breakpoints, stepping or a running session behaves differently, and a client that never sends the request sees no change at all.
